GETTING STARTED
Now that you have read the introductory pages of my Web-Site, you may wish to enter into a psychotherapy relationship with me. Please read and consider carefully the following: FEE SCHEDULE: For Face-to-Face Psychotherapy at my office locations, please contact me for more information on fees, co-pays, and self-pay. For ethical and legal reasons, fee schedules can not be published. INFORMED CONSENT: At the initial session, you will receive a copy of the Informed Consent document which will be explained to you. You will also receive information on expectations of those who enter into counseling relationships with me. You will have an opportunity to share your expectations of me and therapy with me as well. At this time, I accept clients with: ComPsych EAP, Anthem EAP, Humana, APS Healthcare, Blue Cross/Blue Shield, Ceridian EAP, Aetna, United Behavioral Healthcare, and Cameron and Associates EAP. CONFIDENTIALITY: It is vital that you believe and are assured that nothing you share in counseling/therapy sessions will be shared with anyone for any reason except those explained in the Informed Consent. All files are kept in a locked cabinet and are only seen by me. Files must be kept for seven years. By law and ethical mandate, I can only break confidentiality if I believe you are a danger to yourself, if child abuse is reported to me, and if vulnerable adult abuse is reported to me. If you chose to sign away your rights to confidentiality, please know that I must answer questions and share therapy session information in a court of law. METHODS OF PAYMENT: I accept insurance and self-pay and you may use checks, credit cards, and/or cash. You may use PayPal and the icon for PayPal is on the front page of my web site. I have a PayPal virtual terminal so that credit cards may be used. |

THE NEXT STEP After completing the INFORMED CONSENT FORM, the PERSONAL INFORMATION INTERVIEW, and any other necessary forms in my office and letting me know whether you want to use your insurance option (I will give you Jean's phone number. She handles my insurance billing), we can begin exploring your goals for counseling and different methods and techniques which I use. I use experiential methods including EMDR, ego state theory and DNMS therapy, Gestalt techniques, DBT exercises, and many more calming and therapeutic strategies. Please begin to think about what you would like to gain from counseling in terms of goals, changes, hopes and dreams, as we work together.
